Coaches need to have 1 representative for their team check in at the tournament

registration tent located in the center of the park at Canton Sports Center.

 

1. Copy of Team Insurance

2. Compuware Classic Tournament Waiver Form signed by all participants/parents (minors).

3. All Coaches participating with your team must turn in a copy of a competion certificate from one of the below approved concussion courses. You must also turn in the Team Concussion Form sign by players and parents found below under Concussion Form.

Beginning June 30, 2013, Michigan lawmakers passed a law stating that all adults associated with youth programs and all participants under the age of 18, must review a Concussion Training program.

Adults: 

All adults associated with youth programs must complete a training program that has been approved by Michigan Law. The two web sites are listed below:

Either of these sites will take you about 30 minutes to complete and there is no cost. Once you have completed the course, you will be able to print a certificate. 

If you have a certificate from either of the above listed web sites, this will satisfy the requirement. 

 

Players:

All players must review the concussion requirements, then print and sign their name on the Team Concussion Form below.

 

Concussion Form:

This Concussion Form, which is now a Fill-in Form must be completed by all adults and players, and then submitted at check in. The fillable columns are Student Athletic Name and Parent or Guardian Name only!

 4.  Verify your email and phone upon arrival as well.
